Notice:
This is an application for grease interceptor or grease trap permit and does not give the right to conduct business or make alterations to the structure (alterations require a permit).
A City Business License and Certificate of Occupancy is required.
A Grease Trap Service Manifest is required to be submitted to the Code Enforcement Division upon servicing the interceptor or trap.
It is the business owner's responsibility to ensure compliance with the requirement to submit grease service manifests; businesses should not rely solely on their service provider.
The business owner shall allow City personnel access to inspect the interceptor or trap for service and maintenance standards.
For every new permit, the service frequency will be every 90 days and at the discretion of City personnel. requests for a change in service frequency requires a written recommendation from a licensed service provider.

1. Business owner acknowledges the service frequency and will ensure timely submission FOG Service Manifests to Code Enforcement
2. The business will ensure the interceptor/trap is maintained in good working order at all times.
3. Business owner is aware that failure to provide regular and timely proof of interceptor/trap maintenance service may result in one or more of the following: Assessment of penalties and fines and disconnection from the City sewer.
4. The business must have a 24 hour emergency contact or service provider that is capable of responding to overflows, cleanups, or repairs.
5. Business owner acknowledges the use of food grinders or additives to emulsify or chemically treat FOG is prohibited.
6. Business owner acknowledges the disposal of waste cooking oil into the plumbing system drainage pipes is prohibited.
7. Business owner acknowledges all drainage pipes in food preparation areas must have a screen installed.
8. The business must establish best business management practices and maintain records of employee training related to handling FOG, including but not limited to dry wiping of cooking utensils prior to washing, proper disposal of cooking oil, maintenance of kitchen exhaust filters, proper disposal of food waste and solids into plastic bags.
