San Bernardino County COVID-19 Vaccination Information

All San Bernardino County residents age 12 and older are eligible to receive the COVID-19 vaccine.

The Pfizer vaccine is the only vaccine approved for persons under 18.  The Pfizer vaccine is being administered at County and State OptumServ sites.  A parent or guardian must accompany their children ages 12 through 17 to vaccine appointments, and a drivers license or birth certificate is needed to verify age.

San Bernardino County has a vaccination site located at Green Tree Golf Course, 14144 Green Tree Boulevard. Entrance to the site is located on the east side of the building near the back patio.  ADA access is available at the front of the building.
