Updating your Business License

If you need to update information about your Business, click here to print out and complete the Business License Update Form.  Updates include changing your mailing address, contact phone numbers and email addresses only.

Modifying your Business License

If the updates you need to make are not listed on the Business License Update Form, then you may need to modify your Business License.  Click here to print out and complete the Business License Modification Form.  Modifications include Business Name changes, adding/removing Partnership personnel and vehicle/personnel changes outside of renewal time for Solicitor and NEMT businesses.

If the updates you need to make are not listed on the Modification Form, then you may need to apply for a new business license.  Please contact the Business License Division for more information.

Cancelling your Business License

If you are no longer doing business and would like to cancel your Business License, please click here to print out and complete the Business License Cancellation Form.

The completed form(s) may be mailed, faxed, or emailed to the Business License Division.
